Our Part A with the Part B is the complete RCFE licensing application process required by the Department of Social Services. It includes the LIC or Part A forms and the longest, most detailed portion of the application, the RCFE Plan of Operation or the Part B.

We are able to complete many of the Part A forms without input from the applicant. However, there are forms requiring direct input from the applicant because the forms require personal and financial information. We take that information and ensure it is included and professionally typed and reviewed. The Part A forms include all forms listed in the RCFE Application Instructions, revised June 2024. (Despite the recent update, there are still requirements missing. We will ensure all information and forms are completed.) The forms include LIC 200, 215, 308, 309, 400, 401, 401a, 402, 403, 403a, 404, 500, 501, 503 (completed by a medical professional), 508 OOS, 606, 610E, 613C-2, 999 (sketches completed by applicant), 9054, and 9282.
